An opportunity has arisen for a dynamic individual to join the Mental health team as a Social Care Practitioner. The post will involve joining a multi-disciplinary team and providing person centred, outcome focussed support to citizens across Denbighshire. The post holder will receive support from a dynamic and responsive team, covering both North and South Denbighshire. The role will be located at Russell house, Rhyl working alongside social workers and support workers supporting adults and their families.
This is an agile working post and will have the opportunity for both office and home working. Appointment subject to Disclosure & Barring Service Check and satisfactory references.
