Job Description

An exciting opportunity has arisen for a Band 5 Community Nurse based within the Home First Team in Pembrokeshire. 

The role will involve supporting the management of a delegated caseload within the Enhanced Bridging Service as part of the Home First Team. This will include caseload management, as well as support and guidance being provided to a team of Band 2 Community HCSWs.

The post holder will ensure high standards of care are delivered to patients and their families/carers within the home environment by ensuring care is assessed, planned, implemented and evaluated.

Main duties of the job

The post holder will be expected to liaise with staff, patient/carers and the wider multi-disciplinary team. It is expected that appropriate delegation of duties and risk management strategies are applied in order to maintain effective communication between those involved in the patient’s care package. 

Pembrokeshire’s Community Nursing Teams have a wide-ranging nursing skill mix and pride themselves on supporting professional development and improving high standards of community practice in line with current Health Board values.

There is currently 1 full time permanent position available, working 5 days over a 7-day period, including weekends and bank holidays.  

The post holder will need the ability to travel across sites in a timely manner.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
