Salary: £8.21 per hour (unqualified) and £8.45 per hour (qualified*) for direct support.
Enhanced rate for unsocial hours £9.88 per hour (unqualified) and £10.44 per hour (qualified*) between 20.00 and 08:00 Monday to Friday and all-day Saturday & Sunday.
*Qualified = NVQ / QCF Level 2 or higher.
Mileage & Travel time: Mileage 37p per mile (from home, in-between and return). Paid travel time.
Based at: Llandrindod Wells, Builth Wells, Rhayader, Knighton and Presteigne
Responsible to: Registered Regional Manager
Main Relationships Internal: Hafal Crossroads staff, Hafal Crossroads members
Main Relationships External: Hafal Crossroads Staff, Hafal Crossroads Members, Individual Service Users.

Job Purpose:

Under the direction of the Regional Manager, Hafal Crossroads operates domiciliary care services including domiciliary home care service, palliative care service and respite service for Carers. In line with our commitment to principles of empowerment, self-management, wellbeing and recovery and ensuring compliance with internal and external quality standards inspection and regulation (including CIW).

About this job:

The service will be delivered in people’s own homes and within their local communities and will respond flexibly to the assessed needs of the referred individual through the provision of tailored packages of care promoting individual’s wellbeing, rights and independence – in some instances this may be the provision of support overnight.

Main Duties and Responsibilities:
All Hafal Crossroads Support Workers will have and be given the qualifications, experience and competencies commensurate with the levels of care they are expected to provide and in accordance with the National Minimum Standards for Domiciliary Care

This includes supporting individuals if required with:

  • All aspects of personal care
  • Mobility and transfers – all staff are trained in accordance with the All Wales Manual Handling Passport.
  • Administration of prescribed oral medication - all staff must complete Agored Cymru accredited Medication Training


  • Completion of detailed observations and reports in relation to individuals which can be shared with health professionals to ensure adequate care arrangements are in place from statutory/voluntary services to meet the individuals needs if continued care and ongoing support is required.
  • Maintaining records and statistics required to monitor and evaluate the effectiveness of the service
  • Communicate effectively with Health Care Professionals, families and Carers
  • Continuous professional development, attending training courses and other as required.
  • Carrying out all reasonable tasks commensurate with the post as directed by line manager
  • Implementing the Hafal Crossroads Equal Opportunities Policy and all other Hafal Crossroads policies and procedures
  • Working to a rolling shift pattern – including mornings, evenings and weekends.
  • To be committed to safeguarding and promoting the welfare of vulnerable adults and children.
