At Case Management Cymru we provide highly professional, bespoke case management to adults and children with a range of life-changing conditions/injuries.
We pride ourselves on delivering a specialist service that recognises the uniqueness of every client; all with very different aspirations and challenges.
We are looking for a warm, patient and reliable individual to work alongside the family to support and facilitate the independence of a 11-year-old boy with an acquired brain injury. He enjoys swimming, cycling and gardening.
Having excellent communication skills and maintaining a consistent and proactive approach, you will be part of the team providing personal care, support him at school and help him follow therapy programmes and social and leisure outings.
The Support Worker will also work with independent therapists in enabling the client to participate in effective neuro rehabilitation.
The successful candidate will encourage and support him to access all these opportunities and more (ability to drive the Clients van is essential).
Whilst experience in this area of work is preferred, it is not essential as a full induction and training will be provided.
This role is subject to an Enhanced level DBS check.
