At Case Management Cymru we provide highly professional, bespoke case management to adults and children with a range of life-changing conditions/injuries.

We pride ourselves on delivering a specialist service that recognises the uniqueness of every client; all with very different aspirations and challenges.

We are looking for a warm, patient and reliable individual to work alongside the family to support and facilitate the independence of a 5 year old girl with Cerebral Palsy.  She enjoys playing with her toys, crafts and swimming

Having excellent communication skills and maintaining a consistent and proactive approach, you will be part of the team providing personal care, helping her follow therapy programmes and social and leisure outings

We are looking for support before and after school. At times, it may be necessary to work additional hours depending on the activities that the client wishes to attend.

The Support Worker will also work with independent therapists in enabling the client to participate in effective neuro rehabilitation.

Our client currently attends her local school.

The successful candidate will encourage and support him to access all these opportunities and more (ability to drive with use of a car is essential)

Whilst experience in this area of work is preferred, it is not essential as a full induction and training will be provided.

This role is subject to an Enhanced level DBS check.
