At Case Management Cymru we provide highly professional, bespoke case management to adults and children with a range of life-changing conditions/injuries.

We pride ourselves on delivering a specialist service that recognises the uniqueness of every client; all with very different aspirations and challenges.

We are looking for a warm, patient and reliable individual to work alongside the family to support and facilitate the independence of a 11 -year-old boy who sustained a brain injury as a baby. He has complex needs including physical, cognitive and behavioural.  He requires support on a 2:1 basis.  He enjoys films, swimming and caring for his rabbits.

His care needs are complex and the family need support to assist the client in his morning routine and travel to and from school.  You will then support him to engage in leisure activities and to use local amenities such as the leisure centre and horse-riding facilities.  At times, it may be necessary to work additional hours depending on the activities that the client wishes to attend.

The Support Worker will also work with independent therapists in enabling the client to participate in effective neuro rehabilitation.

The successful candidate will encourage and support him to access all these opportunities and more (ability to drive with use of a car is essential)

There may be the opportunity in the future to accompany the client on family holidays.

Whilst experience in this area of work is preferred, it is not essential as a full induction and training will be provided.

This role is subject to an Enhanced level DBS check.

To apply for this Support Worker role in Cardiff, please submit your current CV and contact details online.
