Team Leader/Support Worker – one position (Ref CB3)

Contract: Permanent

Location: Cardiff

Hours: 29 Hours per week

(1 x 24 hr shift as Support Worker and 5 hours as Team Leader)

Pay: £12.00 p/h for both SW & TL

At Case Management Cymru we provide highly professional, bespoke case management to adults and children with a range of life-changing conditions/injuries.

We pride ourselves on delivering a specialist service that recognises the uniqueness of every Client; all with very different aspirations and challenges.

We are looking for a warm, patient and reliable individual to support a young man with Cerebral Palsy to live independently within his own home.

Having excellent communication skills and maintaining a consistent and proactive approach, you will be flexible, reliable and enthusiastic in your approach.

Our Client currently attends activities such as Pedal Power and visiting the cinema. A key part of the role is to encourage our Client to explore and attend further social and leisure facilities within the local area as well as promoting his independent skills in his daily living.

The successful candidate will encourage and support him to access all these opportunities and more. The ability to drive using the family vehicle is essential.

In addition, there are five team leader hours per week available which are dedicated to assisting the Case Manager; being responsible for supervision of staff, co-ordination and liaising with others involved as well as rota planning.

A full induction and training will be provided.  

This role is subject to an Enhanced level DBS check.
