At Case Management Cymru we are able to provide a highly professional and individualised specialist case management service to adults and children with a range of complex serious injuries.

We pride ourselves on providing bespoke services, recognising the uniqueness of every client; each having very different aspirations and challenges.

We are looking for a warm, patient and reliable individual to support a man in his 20’s who sustained a brain injury several years ago.

Having excellent communication skills and maintaining a consistent and proactive approach, you will be a key team member assisting our client to access community amenities and activities; prepare meals and undertake other tasks of daily living; continue to participate in his rehabilitation and therapy (ability to drive with use of a car is essential).

Supporting our client to live as independently as possible within his own home will also require you to undertake some cleaning duties and household management.

Whilst experience in this area of work is preferred, it is not essential as a full induction and training will be provided.

This role is subject to an Enhanced level DBS check.
