Join the Newport Support Partnership and provide support and care to a range of vulnerable people across Newport in their own homes and in the community.

These positions have the same responsibility as our contracted staff, however as a relief member of staff, you can select shift patterns that match your personal needs.

What is special about this opportunity?

This role will be crucial in supporting people within the community by providing support to help them achieve positive outcomes in their lives. The services will be provided in collaboration under the partnership model between Reach, Dewis CIL, CAB, and Volunteer Matters.

The support and care will be delivered to a range of vulnerable people across Newport in their own homes and in the community – including the provision of respite response service for carers. It will also offer the Appropriate Adult response service.

For this role you will require an Enhanced DBS check, which we pay for.

How can I make a difference?

You will support people to:

  • Be part of their communities, have opportunities to be as active as possible and access community based activities;
  • Carry out wellbeing checks to vulnerable people to make sure they are safe and ok;
  • Help with basic domestic tasks such as shopping, to help speed up and facilitate hospital discharges;
  • Help with medication management and collection of prescriptions;
  • Make sure people are well, and support citizens in attending health appointments;
  • By undertaking simple household tasks and encouraging service users to do these too;
  • Support people to develop and learn new skills to help them live independently.
  • Working in collaboration with volunteering and befriending services to promote positive wellbeing.
  • Provide personal support plans and their implementation, reviewing their progress and negotiating any further support requirements.

Who are we looking for?

Someone who puts people we support at the forefront and can deliver innovative and flexible support services to help people and their carers have positive wellbeing.

  •  Ability to support people with a variety of needs in the community.
  • Commitment and willingness to support people to develop and maintain supportive links and network within their communities.
  • Work to in a Person Centred way, and effective partnership working with volunteering and befriending services to offer a seamless service to people in the community.
  • Ability to communicate effectively both verbally and in writing with colleagues at all levels and on a multi-agency basis.
  • Ability to work flexible patterns of work including mornings/evening/ and occasional split shift in line with the people reach supports changing needs.
  • A driver with own transport is essential.
