Age Connects Cardiff and the Vale is looking to recruit the following posts to work within the Integrated Discharge Service (IDS) at the University Hospital of Wales and Llandough Hospital. These posts are funded until March 2019.

Frailty Discharge Liaison Officer – x2 posts 35 hours £19,658 *

21 hours £11,795

These newly created roles will be based in the Frail Older Person Advisory Liaison Service (FOPAL) within the Emergency Unit (EU) at the UHW to assist in the facilitation of safe discharge planning for people over the age of 60.

All posts will need to work flexibly as part of a large integrated team of health and social care professionals in a busy environment.

*Part-time hours may be considered

The posts will be subject to a satisfactory Enhanced Disclosure check.
